Application for Absence for Exceptional Circumstances

All 'Applications for Absence for Exceptional Circumstances' need to be notified to the office as far in advance as possible but can only be approved at the discretion of the Headteacher in exceptional circumstances.

Permission to Leave School Form for Year 5 & 6 only

Children in Y5 and Y6 are allowed to leave school unaccompanied via the Sheerwater gate. provided a permission form is submitted in advance.

Allergies & Special Diet Requests

Twelve 15 cater for 14 recognised food allergens: celery, crustaceans, dairy, egg, fish, gluten, lupin, molluscs, mustard, nut, peanut, sesame, soya, sulphites. They also support vegetarian, vegan, pescatarian, pork, beef free diets. If your child requires any of these diets and would like a school meal, please make sure you have informed us in writing of their special dietary requirements. (

Health Care Plan & Medication Permission

Please complete and return a Medication Permission form should your child require medication administered whilst they are in school.

If your child has a medical condition or requires regular medication, please complete the attached Health Care Plan.

Free School Meal Eligibility & Pupil Premium Application Form

Your completion of one of these forms enables the school to check for Free School Meal Eligibility and also if additional money (Pupil Premium) can be claimed from the Government. Free School Meals can be provided to pupils if parents/guardians meet the eligibility CRITERIA 1 overleaf. We urge you to complete this form even if your child does not take a school meal.
